Integrating Transition Goals and the IEP
Steps to ensure transition planning is addressed
1. Person-centered planning session - This is done by one-on-one interviews with the student, family members, and other stakeholders. Dislikes, likes, fears, and student's visions and dreams should be discussed.
2. Responsibilities of the stake holders - They should agree to assist and carry out the steps that bring the student closer to their goal.
3. Student-led meetings - These help the student develop self advocacy skills, actively plan for their present and future goals, practice communication and negotiation skills, makes student responsible for their decisions, helps with self-evaluation and self-management skills, and helps them participate in a team approach to problem-solving.
4. Integrating transition goals into the overall goals of the IEP - A link needs to be made from the transition goals to identification of what skills and knowledge the student needs.
5. Respecting cultural diversity - Cultural heritage, family traditions, and cultural norms should be considered when writing final goals and benchmarks.
